BSW Posting Guidelines

This forum is a private log for all Banned, Suspended and Warned members.
Effective from today (6/28/15) all posts in the BSW section should contain the following information:
1) The Username that action is being taken on.
2) The Rule that was violated/what the member did
3) The action taken against the user
This should result in a more consistent moderation policy across the forums.
The Username(s) that action is being taken on shall also be the subject of the thread - one thread per member please. (In the case of PBU's, one thread per user, with all usernames being listed.)
As an example (from TSR):
Username: spotify95boston
Reason: Impersonating a user to cause distress and derail a thread.
Action: Banned.
